Clinical Case Manager
Location: Fully Remote
Pay: $52.75/hr
Description:
Support and ensure that patients receive the appropriate services and help manage resources, reducing unnecessary hospital readmissions.
Responsibilities:
- Assess patients' medical, emotional, and social needs to create a comprehensive care plan.
- Collaborate with healthcare providers, families, and other stakeholders to ensure the patient receives high-quality care.
- Monitor the progress of patients and adjust care plans as needed to achieve optimal outcomes.
- Educate patients and their families on treatment options and health management strategies.
- Act as an advocate for patients, ensuring they receive necessary medical services in a timely manner.
- AProvide guidance and support in navigating healthcare systems, including insurance and referral processes.
Qualifications:
- Bachelor's or Master's degree in Nursing, Social Work, or related healthcare field.
- Current nursing license or social work certification (depending on the specialty).
- Minimum of 3-5 years of experience in case management, clinical care, or a related field.
- Strong knowledge of healthcare regulations, insurance policies, and patient advocacy.
- Excellent communication, problem-solving, and organizational skills.
- Ability to work independently and as part of a multidisciplinary team.
